Evergreen Community Inc., established in August 2012, is a non-profit organization. The primary aim is to provide aged care services for people aged over 65 and above, from different cultural background and with function limitations. We developed a wide range of activities that are tailored to the special needs and physical conditions of the elderly people in order to promote the Australia’s “living longer, living better” ageing policy. At the current stage, more than 700 elderly and disabled people around Brisbane areas are registered for our services. We wish every clients would feel happy and welcomed during their time at Evergreen Community Inc.

PROGRAM AND SERVICES

A： Wide range of activities are provided:

·         Enhance physical fitness and disease prevention activities: Joint exercise, Tai Chi, Dance Classes, Health Seminar

·         Improve self-confident and leisure activities: Beginner English Class, Basic Computer Class, Singing Group, Playing     Games

·         Brain training exercises that may delay memory deterioration: Art and Craft, Flower Arrangements, Origami, Mahjong, Playing Cards, Bingo

·         Social group activities：Birthday Party Celebration, Outings, Festival Lunches, Yum Cha, Community News Reading, Dance ball

C: Services we are planning to provide in the future:

·         Personal Care: Eating, Bathing, Toileting, Dressing, Grooming, Getting in and out of bed and Moving around the house.

·         Social Support Visiting Services: telephone based monitoring services and assisting the person with shopping and other related activities

·         Client Care Coordination: Implement the care plan; liaison within, or other service providers; monitoring and reviewing the care plan or service plan.

·         Home Modifications: Grab and shower rails; appropriate tap sets; hand rails; raps and other mobility aids; installation of emergency alarms and other safety aids and other minor renovation

·         Support services: provision of meals, occasional care and referral services
